Multiple addindex commands in HTML with same name

It is possible to specify an addindex multiple times. In HTML this resulted in
- a 'a' tag with with the same 'name' attribute
- in CHM only one occurrence was shown.
this has been corrected.
(see e.g. the index of 'flex' in the CHM manual generated ffrom master).

Analogous to the `\htmlonly` also with the `\htmlinclude` command it is possible that some parts are added that cannot be inside a `<p> ... <\/p>` and thus we have to temporary close (and later reopen) the paragraph.
The option `[block]` has been added to the `\htmlinclude` command (analogous to the `\htmlonly` command).
Problem can be seen with the default doxygen test 30 (`[030_htmlinclude.dox]: test the \htmlinclude command`).

Added underline possibility and strike through possibility for the different output formats insofar it is possible (other similar possibilities are not always possible for all output formats either).

The alt attribute is mandatory for the image tag, it is possible to have it written out with an empty string.
In case with the image tag there is no alt attribute the empty string alt attribute is added.

Commit 62f09103b77b9e5b4093d248563e2f9d76b65728 which
merged pull request 538 introduced a bug in the generation
of plantuml diagrams for other plantuml sources than the
htmldocvisitor. In the previous commit the baseName argument
was wrongly interpreted as file name without path. This
commit fix this issue so the behaviour should be the same
as before, while still also having the support for
PlantUML diagrams in Qt compressed help files.

Plantuml generated diagrams will be included in the
index.qcp file which defines what files to include
in the Qt compressed help file. This means that
plantuml diagrams will work in the Qt creator help
viewer.

This patch makes the handling of the \snippet and other commands consistent between the different languages (no line numbers anymore with python) and also introduces analogous to \includelineno the command \snippetlineno.
Some non relevant changes:
- *code.l Calculation of the end line was incorrect, in case of a snippet the end line was the number of lines of the snippet and not reltive to the start line.
- *code.l made consistent over the different laguages, enabling exBlock and inlineFragment
- testing/indexpage.xml in test 14 the \snippet command was used with python and giving line numbers, linenumbers are now gone (consistency)

Based on the request http://stackoverflow.com/questions/27036474/how-do-make-doxygen-generate-index-color-png-files for index colored png files, this functionality will be included in the newer versions (2.39) of dot.
The functionality -Tpng:cairo:gd will automatically be used by doxygen, though the file extension would be wrong (.png:cairo:gd).
With this patch the :cairo:gd is stripped from the image extension, but not from the -T of the dot command.
In the doxywizard the different possibilities are supplied as well. Note however that the DOT_IMAGE_FORMAT name is not checked when read from the Doxyfile so it is possible to use other renderer / formats as well due to the generic solution with getDot ImageExtension.

This patch adjusts some problems regarding image sizes in LaTeX output of doxygen (a.o. Bug 738299 - When using msc or PlantUML, the default latex for the created image should include [width=\linewidth]) this has been done is such a way that all the "image" commands (i.e. image, dot, dotfile, msc, mscfile, diafile, startuml) operate in a similar way / have the same possibilities.
