When a hyperlink splits across a page boundary it can come to the problem of "\pdfendlink ended up in different nesting level than \pdfstartlink".
To overcome this problem the \hyperlink is packed into a "\mbox" construct (as suffested e.g. in https://tex.stackexchange.com/questions/1522/pdfendlink-ended-up-in-different-nesting-level-than-pdfstartlink)

When having a line like "Definition at line 30 of file test.cpp." in the LaTeX document with source code the test.cpp is linked to the file but the 30 isn't (in HTML it is).
Problem is that in the hypertarget the extension is still present. This patch corrects this.

In the documentation generated by doxygen the table of contents does not start with page number i but with iii, by setting the pagenumbering}{roman} after the clearemptydoublepage titlepage this is corrected. In the beginning of the documentation we set the page numbering to "alph" so it won't interfere with the numbers in the table of contents and the real documentation.
For the doxygen manual similar reasoning is true, furthermore the table of contents was on the left side page (when printed double sided), yhis is corrected as well.

This patch adjusts some problems regarding image sizes in LaTeX output of doxygen (a.o. Bug 738299 - When using msc or PlantUML, the default latex for the created image should include [width=\linewidth]) this has been done is such a way that all the "image" commands (i.e. image, dot, dotfile, msc, mscfile, diafile, startuml) operate in a similar way / have the same possibilities.

When using the mathcal command in formulas the mathptmx package gives in the different output version different letters.
When the mathptmx, more exuberant, type of fonts are to be used they can be added by means of the EXTRA_PACKAGES option for the dot version.

- Bibliography refers to wrong page in the toc
In case of LaTeX the reference in the toc tab to the Bibliography page is not correct.
According to e.g. http://lookherefirst.wordpress.com/2008/10/05/add-the-bibliography-in-the-table-of-contents/
when using the hyperref package the reference of the toc should be after the \bibliography entry.
See to it that the Bibliography page starts at an odd page (like all other chapters)
- See to it that the Index page starts at an odd page (like all other chapters)
- Not all items are shown in the Index / Index generation crashes (Bug_733323) / Index is not shown
The \hypertarget definition contains on 2 places the entire string (including section references and index references) in the caption part.
In all other places the caption is empty and the caption part is not used (as a reference) in the \hyperlink command. The caption has been made empty/

In doxygen a limited number of HTML4 special character entities are recognized, with this patch the full list of HTML4 special character entities (see http://www.w3.org/TR/html4/sgml/entities.html) is supported (as far as they can be represented in the output format).

Mapping (for recognition of the entities) based on running through the list of available entities, after validation of the order of the entities. A few entities contain now besides letters also numbers at the end.

Define symbols for all entities as well as a helper struct and enum. The member m_letter is not used anymore and thus removed.
All existing symbols were renamed so no confusion (i.e. errors when forgetting to rename some symbols) can occur.
Obsolete references to letter removed, all letters with accents etc. are explicitly defined and not through the generic accent name plus an extra letter.

In the doxygen manual the index items for \{ and \} were missing due to the missing support for the usage of { and } in parts of the code.
This patch fixes this problem by introducing 2 new latex commands. See also http://tex.stackexchange.com/questions/153291/index-unmatched-braces-in-latex
Further improvements in the index are:
- consistency in different places
- correction of index for \::
- placing \~ on a more logical place (together with other special characters, ~ is in the ASCII table after a-z whilst other characters are before this range)
