When a gradient was set on the whole window then there was a line above
the menubar that repeated the gradient for each menu item. Since there
is a workaround in place already for the background of the menu to get
painted correctly, we just extend the rect this covers to ensure the
extra line of pixels is included.

Some changes was done on Mac to support Qt building statically
on Cocoa. Unfortunatly, the change lead to many visibility warnings
because, with the change, src/tools/bootstrap was made with symbols
hidden by default (-fvisibility=hidden). Then, when building the
tools (moc, rcc etc), visibility was public by default (for internal
classes used by Qt). This caused warnings. The change is now temporarily
reverted until Morten get a second chance to look at it.

We only want to expand the width of the text, never shrink it. This is
so that we can account for text where the pixels of the text extend
beyond the edge of the last glyph. When we shrinked the width, code
that depended on the natural text width equalling the advance of the
text (such as WebKit) broke, and some text items would be positioned
wrongly. We now only take negative right bearings into account (right
bearing is left-bound), meaning that we only expand the text width,
never shrink it due to right bearing.

This is useful when animating the blur radius, as you don't want to
suffer the hit of compiling / linking a new shader program for each
radius the first time the animation is played.
Also use the fast blur when the radius is 5 or below, as the quality
difference is insignificant.

With this patch, the application developer can use his own
CEikApplication, CEikDocument and CEikAppUi classes with Qt, by
deriving from QS60MainApplication, QSMainDocument and QS60MainAppUi,
respectively. He can then register a factory function in the
QApplication constructor to have his own objects created during the
framework initialization.
This patch also fixes some Qt code style issues.
