- Replace Nokia contact email address with Qt Project website.
- Remove "All rights reserved" line from license headers.
As in the past, to avoid rewriting various autotests that contain
line-number information, an extra blank line has been inserted at the
end of the license text to ensure that this commit does not change the
total number of lines in the license header.

The implementation of hfw from this patch is unfortunately not robust enough.
It doesn't manage correctly the constraints on the layouts and the cell spans.
It caused bad behaviour or regressions seen in tasks:
QTBUG-13547, QTBUG-13067, QTBUG-13549, and more

The reason was that the row/column count was not updated after an item
was removed. (Note that qgraphicslinearlayout already did this, so we
just follow the same pattern, except that the code for
QGraphicsGridLayout is a bit more complex...

We move the QLayoutStyleInfo class out of the gridlayout engine to a
common header file so that anchor layout also can utilize it.
Due to that we now can have a per-item spacing we have to change the
'effectiveSpacing' argument of refreshSizeHints to just take a
QLayoutStyleInfo pointer that we will later query for the actual
spacing used.

This means it should respect the order that was defined with
insertItem() and addItem().
Note that this is not strictly necessary (as now explicitly written in
the docs for QGraphicsLayout::itemAt()), but commit
2ec56d158dc140f68efb45e2e0613f0e4026ddf6 broke the order and for people
that relied on this that commit caused a regression.
In addition, after commit 2ec56d158dc140f68efb45e2e0613f0e4026ddf6 it
was not longer possible to query the "item at visual index". Thus,
instead of adding another function (like QGGL::itemAt(int,int)) we make
sure that itemAt() also returns the "item at visual index".
