The QGtk class has been refactored into a private d class for QGtkStyle.
This allows us to re-use a lot of code for the Maemo5 style without changing
a single line in QGtkStyle itself plus we can easily add virtual functions
where the two styles need to behave different.
There shouldn't be any new functionality added (or old functionality lost) by
this commit.

Add a backend for QFileIconProvider in the platform plugin
Also change the QIcon::fromTheme backend in the platform plugin:
On KDE, we unfortunately can't use KIcon as backend, as the current
API doesn't let us know easily (and quickly) wether we should use
the fallback or not (KDE always fallback to the question mark
"unknown" icon)
So we will use the QIconLoader even on KDE.
But we need to make sure that the theme name and the icon search paths
are correct. Ask that to the platform plugin

This adds some code to support native filesystem icons
on GNOME. It works by resolving gnome libs and gnome-vfs dynamically,
hence we are explicitly running it on GNOME only and not KDE.
Even if it would work there as well. We are planning on adding this
functionality to the platform plugin as well.

It turns out that we weren't doing any caching of icons provided by the
file system. We now use the similar trick that's used on Windows which
does some caching on the file extension. We do fill up the cache
needlessly with extra information (16, 32, 64, and 128) icons. We
probably could be better with a iconRef engine that generates these
sizes on demand. Still performance is 100% better with this which means
using it in itemviews works.

This is more follow the cue of what is done on X11, mainly, if you are
creating things like messageboxes or file views, you want them to follow
the desktop (yes, you do). If you disable desktop settings aware, you
get the old look. This also meant shifting around some functions into
qt_cocoa_helpers_mac to make them more readily available instead of
living in differnt files. People who use standard pixmap get the old
values, but I think that's fine. If you haven't moved onto standardIcon
(introduced in 4.1), you don't get the latest bling.

Introduce Q_WS_WINCE for Windows CE only windowing parts. So far we
decided to stick with Q_WS_WIN32, but having a separate define
makes the code more readable. In addition Q_WS_WINCE_WM is available
for Windows Mobile only parts, where we do not check for the OS on
runtime.
