compilation with QT_NO_DEBUG_STREAM
Qt in debug need to stay binary compatible with Qt in release.

QtGui had some debug functions only exported in the debug build.
Now these are exported in release mode as well, but as stubs (i.e. no
debug output is generated).

After discussions with Matthias, several things were changed:
- the autoSipOnMouseFocus property was removed and replaced with a
new style hint, SH_RequestSoftwareInputPanel. This means the style
can define when the input panel is launched.
- The code which sends RequestSoftwareInputPanel events was moved
into its own function, and the widgets call that function.

Rationale: We're using template magic now to get the private
pointer in qglobal.h, so no need to have two macros. Also
keeps backward compatibility with outside (KDE) code.

KDE and GNOME has the concept of a default tool button style that
can be set system-wide. Qt currently allways default to IconOnly.
I have added an optional Qt::ToolButtonSystemDefault value so you
can opt-in to respect the system setting. We did not change the
default because a lot of apps will look odd when for instance
text is beside icons and the descriptive text is too long.

This class adds convenient support for proxy styles in Qt.
Note that to support proper proxying, styles now
call their functions through proxy() so that the proxy regains control
over subcomponents. Note that there is a small price to be payed
by the extra function call which can at the cost of readability
be redirected to the private class member in the future.
