Sort of imbarassing, but baaa5ae went in before I checked

It seems that the change above broke some auto tests, which
revealed a true problem. When it comes to modal dialog, children
still needs to be stacked on top of modal parents, as they the
user cannot use the mouse to raise it. So rather than removing
subWindowStacking fully, we narrow it even further down to
only be used for children of modal dialogs. All in all, this
is close to removing it, but still us it for certain corner cases.

This reverts commit 3c2373d7ea9bc91bb537c0725984d19ad0fbab01.
After finding yet another bug related to cocoa child windows
(QTBUG-17738), we have no other option than to admit it was a wrong
move to use the API in the first place. Had we only known how many
side-effects and hidden bugs it would introduce. The original problem
we tried to solve were the cases where a stays-on-top parent window
executed a modal child dialog. This child should always stay on top of
its parent, but Cocoa would insist on pushing the window down to the modal
window level upon activating/deactivating the application. Some window
systems will always stack a window child on top of the parent, while
others (X11) seems to be more selective on this issue. On Mac, we already
stack windows a bit differently, thinking first and foremost on tool
windows.
Since this change is going into a patch release (which is debatable,
since this changes behaviour, but p1 is a p1), we choose to add in a
backdoor for those users who by chance depend on this behaviour. Setting
the env var QT_MAC_USE_CHILDWINDOWS=1 will give you the old code
path, but we plan to remove this for Qt-4.8.
Also, this patch does fix the original bug described above by overriding
the setLevel method in NSWindow, and refuse Cocoa to level down
stays-on-top modal windows.

The combobox popup on mac fails to re-highlight the last item in the
list when then mouse leaves the popup, and re-enter. We tried to fix
this in 780b4d84, but is turned out to break the cleanlooks style.
For this second try, we allow the code to set the index under mouse
when the mouse hovers an item in the list, even if the index is
current. It seems to cause low overhead, and should be safe.

This problem has been known for a long time, but a good solution has
never been found. The problem is that a child window should always
stay on top of it's parent, if not for anything else than secure that
a modal child does not block input while hiding behind the parent at
the same time. The only sensible solution found to ensure this in the
Cocoa port is to use Cocoa child windows. But this API has a sad side
effect; it will move the child along with the parent when the parent
is moved on screen. This is something it seems we have to live with.
But for those users that wants to handle this issue otherwise, we now
add a widget flag to switch this off.

When using TV-out, the TV display can be used as an independent screen.
By default the content shown there is a clone of the device screen,
however from now on parenting a widget to QDesktopWidget::screen(1)
and calling show() will turn off cloning and have the widget shown
instead. screenCount() and the screenCountChanged signal can be used
to detect the availability of the secondary display, just like on other
platforms.

QNativeImageHandleProvider is a thin interface consisting of get()
and release() functions. Pixmaps constructed with such a provider
will call these functions to acquire and release a native handle,
e.g. a CFbsBitmap or RSgImage pointer in case of Symbian. The behavior
is largely similar to constructing pixmaps via fromSymbianCFbsBitmap
or fromSymbianRSgImage, with the exception of pixmap hibernation:
release() (and subsequently get()) is guaranteed to be called also
in case of hibernation, allowing more fine-grained tracking of
the usage and lifetime of image data.

By default Symbian devices use fullscreen editing mode. However, in the
latest AVKON releases a partial virtual keyboard is supported. Support
the non-fullscreen editing mode from QCoeFepInputContext.
The non-fullscreen editing mode is only supported for Symbian^3 and
only for graphicsview-based solutions (QGraphicsView, QGraphicsWebView,
QDeclarativeView, ...). When native side indicates that the keyboard
opens, the graphicsview is possibly translated so that the text cursor
position is ensured to be visible. When keyboard closes, the
translation is removed. If the graphicsview contains vertical scrollbar,
the whole view is resized to the area above the keyboard, since
translating it, would move the upper part of the scrollbar out of
screen area.
There is a new exported private API to control when partial vkb is
used.

Commit 0ba1b4d0 introduced a regression to QTextDocument: it postponed
cursor position adjustment until the move operation is done, but
contentsChanged will be triggered by finishEdit() in this move
operation, thus cursor positions in this signal handler will be in
inconsistent states (normally we should first update cursor position
then trigger contentsChanged). In this case we should also postpone
finishEdit() handling after cursor positions have been adjusted, then
the states expose to applications will be consistent.

After ensure space, local QGlyphLayout variable should be updated to
the moved address.
stringToCMap returns true for success, so stringToCMapFailed should
be the reverse of the return value.
Out of bounds happened quite often in all situations using the
QStackedTextEngine, because the memory allocated at the first time
is usually not much, making it easier to trigger out of bounds in
shaping. But it can also happen when using normal QTextLayout.
Test it by comparing the width returned by normal QTextLayout and
the width returned by QTextLayout created from QStackedTextEngine
via QFontMetricsF.

The runtime window system has a 'proxy' window surface which wraps the
currently active window surface. When the window geometry changes, the
new geometry is properly communicated to the wrapped window surface.
However, the new geometry is not updated into the runtime window surface
proxy itself, which means that when queried, the geometry for the window
surface will always be invalid. This patch fixes the issue.
