By keeping record of the iteration count of the event loop, we can
determine whether the active object being processed is being run for
the first time or not in the current iteration. This is slightly
faster, since it saves us from adding the AO to the deferred queue
the second time around if the iteration is different. It is also
slightly more correct, since otherwise running a timer twice would
take three calls to processEvents instead of two (first time: run AO,
second time: add to queue, third time: run AO from the queue).

The change was done in order to get a more round-robin type of event
handling, at least for Qt active objects. With this change, each
object will only execute once within each call to processEvents,
like other platforms.
This is required for certain tests using Open C, because some of the
function calls in that library take much longer than their desktop
counterparts. Therefore the timer expires before the previous timer
handler was finished and the test gets stuck executing the same timer
over and over. By returning from processEvents after one iteration,
we prevent this.

Some text drawn with OpenType fonts where cut off by a pixel or two
under certain circumstances. This adds an additional 2 pixel pad margin
to the glyph cache entries. The padding behaves slightly different
when ClearType is enabled/disabled, hence the general 2 pixel padding.

QGraphicsItem::deviceTransform() returns the item-to-device transform,
provided with the device-to-scene transform, and combining it with the
item's scene transform. This function is meant to handle items that
enable ItemIgnoresTransformations, but it happened to not work properly
for items that _don't_ enable that flag. Unfortunately this bug is
hard to work around for users from the outside, as it requires you to
check if the item or any ancestor enables ItemIgnoresTransformations.
The fix also removes unnecessary branchs inside QGV so that we use the
same function for all items.

In QGraphicsScene::_q_emitUpdated() the slot
QGrpahicsView::updateScene(QList<QRectF>) gets connected and a boolean
(connectedToScene) is set to prevent double connections. The problem is
that this boolean was not reset when the view gets a new scene.
