Doc: update msvc, add version table [skip appveyor]
The changes in 4.4 didn't marke new construction variables with the
now preferred version add marker, so added these.
The references to different versions gets so confusing that to try
to help added a version correspondence table to the MSVC_VERSION
construction varaiable doc.
