define_module(): Change the "index sub-item" for definitions at module

                  scope to be " (in module <name>)" instead of
                  " (in <name>)" to be consistent with \withsubitem
                  usage throughout the documentation.  This achieves
                  consistency in indexing throughout the documentation.
