Three other commits are related to this one: r62269 and r62279, which were changes to PyNumber_Index (among other things) to check for nb_int slots when we lack nb_index slots -- and r62292, which is when I reverted these changes after various people pointed out that the test was in fact wrong, not the code.

The most obvious changes: - str8 renamed to bytes (PyString at the C level); - bytes renamed to buffer (PyBytes at the C level); - PyString and PyUnicode are no longer compatible. I.e. we now have an immutable bytes type and a mutable bytes type. The behavior of PyString was modified quite a bit, to make it more bytes-like. Some changes are still on the to-do list.
