[svn-r30112] Description:
Move updated flush dependency code in metadata cache from revise_chunks
branch to trunk. Also many of the cleanups from r30111 in the revise_chunks
branch.
Tested on:
MacOSX/64 10.11.5 (amazon) w/serial, parallel & production
(h5committest forthcoming)
