Several macros for handling haddr_t values exist in H5Fprivate.h
and have H5F prefixes, even though they have nothing to do with
a particular file (e.g., H5F_addr_lt()).
These macros have been moved to H5private.h and renamed to have
an H5 prefix.

This change ends just-in-time initialization of package dependencies by
package initializers.
Don't track in per-package variables (H5_PKG_INIT_VAR) whether each
package has been initialized. Instead, track in a single library
variable whether the whole library is initialized or not.
Drive the initialization of packages by H5_init_library() with a table
of initializer routines. Also drive the termination of packages by
H5_term_library() with a table.
Perform initialization as needed from FUNC_ENTER_API_INIT(err). This
basically restores the old behavior of that macro.
Delete a bunch of #definitions in H5private.h that have fallen out of
use with these changes.

Description:
- Added functions to query chunk information:
H5Dget_num_chunks(dset_id, fspace_id, *nchunks)
Gets the number of written chunks that intersect with the given
dataspace. However, in this version, the intersection is not
yet completed. Thus, the number of all written chunks will be
returned.
H5Dget_chunk_info_by_coord(dset_id, *offset, *filter_mask, *addr, *size)
Given a chunk's logical coordinates, returns the chunk's filter,
address, and size.
H5Dget_chunk_info(dset_id, fspace_id, index, *offset, *filter_mask, *addr, *size)
Given a chunk's index, returns the chunk's logical coordinates, filter,
address, and size. The chunk belongs to a set of chunks that have
nonempty intersection with the specified dataspace. However, in
this version, the intersection is not yet completed, and the index
is of all the written chunks.
These functions comply with VOL.
- Fixed some oversights found in the library for the tests in chunk_info.c
to work correctly. The returned value from a callback function was not
checked in H5EA_iterate(), H5FA_iterate(), and H5D__none_idx_iterate().
This oversight caused a callback function to continue iterating even though
it's supposed to stop.

Complete revamp of package initialization/shutdown mechanism in the library.
Each package now has a single init/term routine.
This new way should avoid packages being re-initialized during library
shutdown and is also be _much_ more proactive about giving feedback for
resource leaks internal to the library.
Introduces a new "module" header file for packages in the library
(e.g src/H5Fmodule.h) which sets up some necessary package configuration macros
for the FUNC_ENTER/LEAVE macros. (The VFL drivers have their own slightly
modified version of this header, src/H5FDdrvr_module.h)
Also cleaned up a bunch of resources leaks all across the library and tests,
along with addressing many warnings, as I encountered them.

Refactor extensible array metadata lookup to abstract common code into
single, reusable routine. Re-target existing 'set' & 'get' routines to use new
