tests. Roll H5FD*__init_package routines into H5FD*_init() routines.
This change ends just-in-time initialization of package dependencies by
package initializers.
Don't track in per-package variables (H5_PKG_INIT_VAR) whether each
package has been initialized. Instead, track in a single library
variable whether the whole library is initialized or not.
Drive the initialization of packages by H5_init_library() with a table
of initializer routines. Also drive the termination of packages by
H5_term_library() with a table.
Perform initialization as needed from FUNC_ENTER_API_INIT(err). This
basically restores the old behavior of that macro.
Delete a bunch of #definitions in H5private.h that have fallen out of
use with these changes.

of my disabling it. Bracket some code in /* clang-format off */ /*
clang-format on */ to prevent a recurrence.

be necessary. Tests still pass when I remove it, so away it goes.

and exclude that file from trace processing because the `bin/trace`
cannot deal with the function-pointer type.

Complete revamp of package initialization/shutdown mechanism in the library.
Each package now has a single init/term routine.
This new way should avoid packages being re-initialized during library
shutdown and is also be _much_ more proactive about giving feedback for
resource leaks internal to the library.
Introduces a new "module" header file for packages in the library
(e.g src/H5Fmodule.h) which sets up some necessary package configuration macros
for the FUNC_ENTER/LEAVE macros. (The VFL drivers have their own slightly
modified version of this header, src/H5FDdrvr_module.h)
Also cleaned up a bunch of resources leaks all across the library and tests,
along with addressing many warnings, as I encountered them.

New feature.
Description:
Add "memory pool" internal data structure. This set of routines is
designed to add a way to allocate small pieces of information for a particular
purpose and then free all the pieces at once (i.e. without having to free each
piece individually). Memory pools are also good for localizing lots of small
allocations that logically belong together.
For example, if you were constructing a temporary linked list, you could
create a new memory pool, allocate all the nodes for the list from the memory
pool and when you were done with the list, just destroy the pool instead of
tracking through the list and freeing each block independently.
