A number of improvements based on a discussion with Chris McCafferty

Add javascript: and telnet: to the types of URLs we ignore.

Add support for several additional URL-valued attributes on the BODY,
FRAME, IFRAME, LINK, OBJECT, and SCRIPT elements.
